CREDENTIALS APPLICATIONS

All Physicians, Dentists, Nurse Practitioners and Midwives applying to work in the Northwest Territories must submit an application for privileges to the Territorial Credentials Committee through the Medical Affairs department of the Stanton Territorial Health Authority. 

The committee reviews all applications and makes recommendations to the hiring Health Authority that the applicant is applying for privileges to. 

The forms for each discipline are located at the right side of this page. Please choose one for your specialty, complete it and submit it to the Medical Affairs department Credentials fax number 867-765-4011.

Note:  If you are a General Practitioner that has an advanced practice in Surgery, Obstetrics or Anaesthesia please open the package for that discipline and print off the applicable pages of procedures for which you wish to apply.
